In a gripping legal battle, the family of a deceased elderly woman is taking on two care facilities over alleged negligence that led to her death. Antoinette Edmundson, acting as the independent executor of Frances Loisi’s estate, filed a complaint on March 31, 2025, in the Circuit Court of Cook County, Illinois against Champion Catalyst for Care, LLC and Abbington Rehab & Nursing Center, Ltd. The lawsuit claims that these facilities failed to provide adequate care and supervision, resulting in multiple falls and injuries for Loisi.
Frances Loisi was admitted to Champion Catalyst for Care’s facility, The Radcliff, on July 30, 2020. According to the complaint filed by Edmundson through her attorneys at Taxman, Pollock, Murray & Bekkerman LLC., Loisi suffered numerous falls between July 2020 and May 2021 while under their care. The plaintiff accuses the facility of violating the Assisted Living and Shared Housing Act by failing to provide necessary assistance and monitoring. Despite knowing Loisi’s risk for falls due to her clinical condition requiring supervision with daily activities like ambulation and transfers, the facility allegedly neglected its duty to ensure her safety.
The complaint details how on June 1, 2021, Loisi experienced an unwitnessed fall leading to severe injuries including fractures that necessitated surgery. It further alleges that Champion Catalyst for Care did not update Loisi’s service plan or adequately assess her needs despite clear signs of declining health. These failures purportedly contributed directly to a deterioration in her condition and increased risk of further harm.
Edmundson seeks judgment against Champion Catalyst for Care in excess of $50,000 citing statutory violations and negligence under both the Illinois Survival Act and Wrongful Death Act. She contends that had proper care been administered as required by law and medical standards, Frances would have avoided significant pain and suffering before her death on May 19, 2023.
Similarly troubling allegations are made against Abbington Rehab & Nursing Center where Loisi resided from June 11, 2021. Here too she reportedly endured multiple falls due to inadequate supervision resulting in serious injuries like hip fractures requiring surgical intervention. The lawsuit claims Abbington Rehab violated provisions under the Nursing Home Care Act by failing to provide necessary care plans or sufficient staffing levels needed for resident safety.
The complaint emphasizes that both facilities were recipients of Medicare funds obligating them legally towards high standards of patient care which they allegedly failed to meet. As such Edmundson seeks damages exceeding $50k plus attorney fees from Abbington Rehab & Nursing Center also citing violations under Illinois law related specifically towards nursing home operations.
